Request changes to your provider profile
To maintain accurate records and keep operations running smoothly, it is crucial that you regularly review your information and notify us as soon as there is a change. Keeping your data up to date ensures that your provider profile and our provider directories are accurate, and that claims, payments, and other requests are processed on time.
To change your business name or update your contact information use one of the following methods:
- CAQH (our preferred method)
- Participating CAQH providers: Log in to your CAQH ProView
- New users: Register for CAQH ProView
- The Carelon Provider Portal
- Log in to our Provider Portal
- Select “Update Demographic Information”
- National Provider Service Line
- Call the Carelon National Provider Service Line at 800-397-1630, Monday to Friday, 8 a.m. to 8 p.m. Eastern time.

Note: Updating a Tax ID requires an accompanying W-9 form. The W-9 can be submitted via the provider portal as an attachment. Providers without access to the provider portal can contact the National Provider Service Line for assistance in submitting a W-9.
To correct an error in our provider directory, use one of the following methods:

- Update your information using CAQH ProView
- Update your information using CAQH ProView

- Log in to our Provider Portal
- Select “Update Demographic Information”

If you are not accepting new patients, complete the Practice is Full Form Statement and fax it to 866-612-7795.
