Effective April 1, 2025, Carelon Behavioral Health will no longer accept new Medicaid provider applications for Northeast Health Partners or Health Colorado, Inc. For further information, please email Coproviderrelations@carelon.com. Existing in-network providers with Northeast Health Partners or Health Colorado, Inc. who wish to add practitioners to their in-network agreement may continue with the enrollment process.
Today we are Carelon Behavioral Health, but when some of these materials were developed, we were Beacon Health Options.
